Management TeamSandi Johnson, Executive Director
Sandi joined the Canyon Creek team in 2005 as the Community and Family Relations Director. Now, as Executive Director Sandi applies her passion for helping residents and families and her experience in the health care field to help guide and give insight in leading a dedicated and knowledgeable staff. Sandi works closely with the Utah Chapter of the Alzheimer’s Association supporting fund raising efforts and caregiver support. In her spare time, Sandi enjoys traveling with her husband anywhere with warm water, spending time with her two daughters, exercising, cooking and reading. Mandi Short , Business Office Manager
Mandy has been a part of the Canyon Creek team since 2007. As Business Office Manager, she is responsible for all of the financial and administrative functions. Mandy also serves as the human resource contact for community personal. She recently graduated from BonLosee Academy of Hair Artistry. She enjoys spending time with her family, shopping, and boating at Bear Lake. Jeanette Chidester, Director of Community RelationsJeanette Chidester comes to Canyon Creek after years of living and working on the East Coast and the Midwest. She joined the Canyon Creek team in February 2006. After starting work here, she soon recognized that working with the residents and their families is truly one of her passions in life. She is actively involved in fundraising to aid in the research for Alzheimer’s. Some of her other passions are getting a chance to spend time with her amazing children Tyler and Brianna, her wonderful husband Eric, fishing, gardening, reading and riding her beach cruiser. Randi Wilson, Community Outreach DirectorRandi is the Community Outreach Director at Canyon Creek Assisted Living and Memory Care. Her core responsibility is to inform health professionals of Canyon Creek and how our care can benefit seniors. Prior to joining the team at Canyon Creek, Randi received her collegiate degree in communications, specifically broadcast and public relations, with a minor in sociology. Upon graduating Randi spent several years working in traditional media. She enjoys working at Canyon Creek and feels very lucky to have found such an amazing community. Last, but not least, she loves each one of her residents. Anne Wolters, RN – Wellness DirectorAnne Wolters brings over 32 years of nursing experience to Canyon Creek. She began her career in a hospital setting, moved into a mental health setting and most recently worked as a home health and hospice nurse. In her free time she enjoys daily meditations and practicing yoga. One of her personal goals is to return to her hobby of wool rug hooking.
Ann Carlson, Kingswood Court DirectorAnn Carlson is the Director of Kingswood Court, Canyon Creek’s Memory Care Community. After her father passed away in 2005 Carlson realized the importance of quality health care. Her new insight motivated her to begin a career within the health care industry. Her experience includes: medical records, skilled nursing, transitional rehabilitation and medical care within a hospital setting. It has become Carlson’s passion to provide the highest standard of care to her residents and make a difference in their lives each and every day. Nikki Wilkins, Personal Preference Coordinator
In her position as Personal Preference Director, Nikki works with the residents to ensure their daily lives are purposeful, and gratifying. She works with new residents, and their families to make the transition as smooth, and comfortable as possible. She has been at Canyon Creek for almost three years. Nikki started out as a Resident Aide in Kingswood Court, moved into the activities department as the Activities Coordinator, and is now the Personal Preference Director. For fun, she enjoys doing anything outdoors, playing the guitar, and spending time with her family. Katelen Perfili, Activities DirectorKatelen started working at Canyon Creek in 2007 as a CNA. After a year of working as a CNA, she moved to Activities. She is very passionate about her job and loves coming to work every day to see the residents. She is married and has a little girl. In her spare time she loves to go on vacations. San Diego is her favorite place. She loves spending quality time with her family, swimming, shopping, and trying new restaurants. Chase Bosh, Director of Dining ServicesChase runs the day to day operations and also the special functions in the kitchen. He has been cooking and baking for over 10 years, and is a graduate of the Art Institute of Salt Lake City. He received his Associates of Science in Baking and Pastry. His passions are cooking and baking, and learning new ways to do it. Also he enjoys spending time with his family and sharing his love of food with others. Brian McGarrity, Director of Maintenance and Housekeeping
Brian oversees and ensures every mechanical part of the building and facilities are in working order. Brian also coordinates the many behind-the-scenes details that make Canyon Creek sparkle and shine. Brian is very proud of his family. He enjoys playing in the outdoors, spending time with family, and volunteering for a good cause. "Sweetie Molly" |
